A Membership Practice
The Center for Health & Wellness strives to deliver the highest level of care for all of our patients. Personalizing each visit is important to patients and providers alike. Establishing a relationship and getting to know each patient is an essential part of being able to effectively treat, educate and empower every patient to become an active player in their health. The current healthcare delivery system of “one size fits all” is completely opposite from our model and our patients can testify to the higher level of attention they receive during their visits. However, continually decreasing insurance reimbursements and rising business costs, have made it almost impossible to maintain a patient-centered clinic. We want to continue to provide exceptional care and, also, provide even more benefits by offering membership to our patients. Members will continue to enjoy more time spent during visits with added benefits of same day/next day appointments, phone and email access to providers and discounts on supplements, injections, IV therapy and medical cosmetics. Members will also continue to be able to use their insurance for visits. However, the membership fee is completely separate and not reimbursable by insurance. Uninsured, cash pay patients will also be accommodated; along with other membership benefits, uninsured members will enjoy a low cost office visit fee of $85 ($250 for the initial visit). Please read the Membership Welcoming Letter and then print Membership Contract, fill out and then mail or bring with you on you next office visit.
